Im testing a trial version of the new version of alcohol 120% and shell extension not work any time if your try load a disc image no apear the option with right-click button and your close windows explorer sometimes and open again shell extension go back to work. I´m testing with Alcohol 120% 1.9.5.3823 and work fine with this version.

You did not post a complete system report, however:

You are NOT testing the trial version at all, you are using an illegal version of Alcohol therefore it might help if you actually purchase it rather than steal it.

Thats my another pc and the same problems shell not work sometimes with build 3823 and i´m buy a version 3105 and work fine.
I´m not say about sptd drive generate a lot of error reported by windows administrative tools.
